* Your confidential healthcare information may be released to
other healthcare professionals for the purpose of providing you with good care.
An example of this is the release of information to a laboratory for the purpose
of obtaining the results of lab tests needed to treat you.
* Your confidential healthcare information may be released to
your insurance provider for the purpose of paying The Barry Robinson Center for
the care you received. An example of this is sending the insurance company
information about your diagnosis and the date you received services for this
diagnosis.
* Your confidential healthcare information may be released for
the purpose of continuing healthcare operations which may include conducting
quality assessment and improvement activities, reviewing the qualifications of
healthcare professionals, and conducting and arranging for medical review,
legal, and auditing services.
* Your confidential information may be released to public
health officials whose job is to improve the health of the community. An example
of this is mandatory communicable disease reporting.
* Your confidential healthcare information may be released to
public or law enforcement officials in the event of investigation in which you
are a victim of abuse, a crime or domestic violence.
* Your confidential information may be released to other
healthcare providers in case you need emergency care.
Your health records are the physical property of The
Barry Robinson Center but you do have rights regarding the information. Please
read carefully.
* You have the right to restrict the use of your confidential
healthcare information unless it is being used or disclosed for a reason that
does not require a consent or an authorization. However, we may choose to refuse
your restriction if it is in conflict with providing you with quality healthcare
or in the event of an emergency situation.
* You have the right to receive confidential communication
about your health status.
* Your confidential healthcare information may be released only
after receiving written authorization from you or your legal representative. You
have the right to take away your permission to release confidential healthcare
information at any time.
* You have the right to inspect and copy your healthcare
information; however; in certain situations we may deny access.
* You have the right to make changes to your healthcare
information.
* You have a right to know who has accessed your confidential
healthcare information and for what purpose.
* You have a right to have a paper copy of the Privacy Notice
if you ask for it even if you have agreed to receive the Privacy Notice
electronically.
The Barry Robinson Center is required by law to
protect the privacy of its clients. The center will keep confidential any and
all client healthcare information and will provide clients with a list of duties
or practices that protect confidential healthcare information.
* The Barry Robinson Center will abide by the terms of this
notice. The center reserves the right to make changes to this notice and
continue to maintain the confidentiality of all healthcare information. Changes
to this notice will be posted and copies may be obtained at The Barry Robinson
Center.
* Your confidential healthcare information may not be released
for any other purpose than that which is identified in this notice or otherwise
required by law.
* You have the right to complain if you believe your rights to
privacy have been violated and you will not be penalized or otherwise retaliated
against for filing a complaint. If you would like further information or feel
