We offer an online substance use screening tool that identifies whether a person has a high or low probability of having a substance use disorder. The SASSI –Substance Abuse Subtle Screening Inventory –is a psychological questionnaire backed by 30 years of research and used by clinicians across the United States and Canada.
This self-assessment screening tool does NOT provide a diagnosis of a substance use disorder, only the probability. It has a 94 percent accuracy rate with adolescents and 92 percent accuracy with adults.
To request a substance use screening for someone age 13-24, please complete the form below.
A clinician trained in using the screening tool will contact you for more information as needed and email a secure link to the screening questionnaire.
After the person completes the questionnaire, we will receive a report and contact you to discuss the results and next steps.
